Minimum quantitiesEach product we offer has a minimum order requirement. The minimum quantity is shown in the first column of pricing for each item. Less than minimum orders may be available on some items with an additional charge. Contact your sales representative at 800-299-3366 ext. 294 for details. Overruns/underrunsHealth Impressions reserves the right to ship and bill up to 5% over or under the ordered amount on custom-imprinted merchandise unless otherwise indicated. Payment and pricingOrganizations with an authorized purchase order may be invoiced with terms of net 30 subject to credit approval. Individuals must remit check or money order with order or use MasterCard, Visa, or American Express. All international orders must be prepaid. Pricing includes a one-color imprint unless otherwise noted. Please note: Additional charges may apply for multicolor prints. Prices are subject to change without notice. Shipping informationAll orders are shipped FOB factory by UPS Ground in most cases. Special ship methods are available for rush orders; ask your sales representative for details. Most orders shipped to Alaska, Hawaii, Puerto Rico, and outside the continental United States, including APO/FPO addresses, are shipped Priority Mail. UPS 2nd Day Air is also available for customers in Alaska, Hawaii, and Puerto Rico. All shipping charges are prepaid by Health Impressions and will be added to your invoice. Shipping addressesBecause many customers have various shipping locations, you might have several shipping addresses to choose from when placing an order. Some of the records have similar or identical addresses but different individual names or departments. Please choose the record with the correct name or department, and type the shipping information as it should read on the packing slip. If the shipping address is the same as the record you chose, you can leave the spaces blank. You have the ability to view the shipping records under your profile at any time. If you notice an error on a record or an individual's name that needs to be changed or deleted, please follow the appropriate link to request a change. ReturnsHealth Impressions will not accept returns without written authorization. Contact your sales representative for details. Artwork informationAll products (unless otherwise indicated) are available with your imprint. You can e-mail your logo to us in TIFF or EPS format. You can also send black-and-white, camera-ready artwork to 5045 Franklin Ave., Waco, TX 76710. If camera-ready art is not available, you may send a business card or letterhead with your logo, and we will create camera-ready artwork for you. Faxed copies and photographs of artwork are generally not acceptable. Multi-piece artwork (for imprints with more than one color) must be coded by color and have proper registration marks. There may be additional charges if artwork needs to be color separated. There will be NO charge for sizing artwork or for setting straight-line type. If you do not have a logo, you may choose any of our stock art at NO additional charge. All artwork will be faxed to you for approval prior to production of your order. Customer approval of artworkIf you do not provide us with camera-ready artwork, our artists will prepare the type or art for you. We will fax, mail, or e-mail a copy of the type or art to you for your approval. We will begin production when you have approved the artwork. Preproduction samplesPreproduction samples are available upon request. You will be billed for charges incurred to produce the preproduction samples. Additional productsHealth Impressions offers many additional products that do not appear on this website. If you do not see what you are looking for, call us, and we will try to find what you need.
